This statement is to reaffirm Advanced Skin Institute's policy on providing equal opportunity to all associates and applicants for employment in accordance with all applicable Equal Employment Opportunity/Affirmative Action laws, directives and regulations of Federal, State, and Local governing bodies or agencies. Advanced Skin Institute will commit the necessary time and resources, both financial and human, to achieve the goals of equal opportunity and affirmative action.

Advanced Skin Institute does not and will not discriminate against associates, prospective associates, customers or vendors, and will make all employment and business decisions in accordance with all applicable federal, state, and local laws prohibiting discrimination.

It is the policy of Advanced Skin Institute to afford equal opportunity regardless of race, religion, color, creed, national origin, sex, age, marital status, disability, veteran status, sexual orientation, status with regard to public assistance or any other characteristic protected now or in the future by government action. This policy applies to all aspects of the application process and employment relationship including, but not limited to, hiring, promotion, transfer, demotion, termination, discipline, and other conditions of employment.

Advanced Skin Institute will evaluate the performance of achieving these affirmative action objectives as well as other established criteria. Any associate who does not comply with the policies and procedures set forth in this statement will be subject to disciplinary action.

Advanced Skin Institute has appointed its Manager, Human Resources (or other delegate) as EEO Coordinator to manage this program. The EEO Coordinator's responsibilities will include monitoring all equal opportunity activities, and reporting the effectiveness of this affirmative action program, as required by Federal, State, and Local agencies. The CEO of Advanced Skin Institute will receive and review reports on the progress of the program. If any associate or applicant for employment believes he/she has been discriminated against, please contact the EEO Coordinator.

Advanced Skin Institute invites all associates and applicants for employment to identify themselves for inclusion in Advanced Skin Institute's affirmative action program.
